Our service is the only hospital-based home healthcare agency in the Martinsville, Henry County and Patrick County area. That means you'll have the same quality and resources of hte hospital available ot you when you need them at home. W'eve been serving patients' home health needs for nearly two deecades and have met the national quality standards set by The Joint Commission.
Our Home Health Services are Available 24 Hours a Day
Registered Nurses provide skilled nursing care in the comfort of your own home and may provide the following:
- Wound Care - including wound vac with wound-certified nurses
- IV Therapy
- Vestibular Therapy
- Congestive Heart Failure teaching and interventions
- COPD teaching, monitoring and interventions
- Post-hospitalization assessment and teaching
- Educaiton regarding care, treatments, disease process, disease management and medication
Home Health Aides are trained in daily activities such as bathing, dressing, personal hygiene and oral care, under the supervision of a registered nurse.
Physical Therapists evaluate, treat and insruct patients whose mobility has been compromised. Education in equipment use, exercsies and home safety are provided to the patient and caregivers ot help the patient live as independently as possible.
Occupational Therapists help individuals achierve independence despite disabilities. Evaluation, treatment and instruction in daily living activities are provided.
Speech Pathologists provide instruction in skills needed to improve the patient's speech, language nad memory function as well as safe swallowing techniques.
Social Workers are available to address the social or emotional needs identified by the patient, their family, their physician and the HomeCare team.
Am I Eligible for Home Health Services?
Our services are available to anyone in need of healthcare at home for an acute or chronic medical condition. Home Health Care is prescribed and directed by the patient’s physician. Hospitalization is not required prior to receiving services.
How Will I Pay for Home Health Services?
Our home care services are a cost-effective alternative to a lengthy hospital stay. For those patients who meet criteria, many insurance companies including Medicaid and Medicare offer extensive home care benefits to their subscribers. Eligibility and payment programs vary with coverage.
What Do I Do Next?
A referral may be made by your doctor, hospital, nursing home or other patient service agency. A free admission screening can be arranged to determine whether home care is appropriate. Screenings can be performed in the hospital, at a long-term care facility or in your home. HomeCare of Memorial Hospital will work with you, your family, and your physician to develop a healthcare plan designed especially for you.
